ESSENTIAL FUNCTIONS:
  • Provide leadership for the Youth Assistance program in reviewing, developing and updating required treatment plans and documentation for youth and families.
  • Demonstrate a comprehensive understanding of cognitive-behavioral intervention concepts and treatment principles.
  • Conduct group sessions in accordance with Care Path manuals.  
  • Advocate and coordinate appropriate referrals addressing, educational, employment, psychological/psychiatric and social needs for youth and family. This includes obtaining and/or monitoring resources. 
  • Complete weekly progress note documentation, within 48 hours of services completed.
  • Ensure that program structure and activities are implemented according to guidelines and expectation.
  • Ensure that each student has appropriate discharge plans including continuing care, if indicated, and has adequate resources required to support ongoing success in the community. 
  • Conduct immediate follow up with youth who are absent from group.
  • Keep YAP program supervisor informed of group members' progress and attendance..
  • Complete and submit monthly reports, expenditure invoices, narrative report, client posters and performance summary report to supervisor. 
  • Attend all staff meetings and trainings.
  • Provide substitution support to facilitators as needed.
  • Be available for crisis intervention.
  • Perform other duties as assigned
 
PREPARATION, EXPERIENCE AND SKILLS:
  • Pursuing Bachelor's/Master's Degree  in Human Services or related field
  • Good written and oral communication skills
  • Knowledge/experience in working with juvenile delinquent population
  • Demonstrated individual and group counseling skills
  • Valid driver's license that meets Starr Commonwealth's policy standards
