What You Will Do:
As our Admission Nurse you will use your talent and abilities in many ways, including:
Consult with and educate the patient/family regarding end-of-life care.
Conduct initial meeting with the patient and family over hospice service and philosophy and obtain consents for care.
Develop an initial care plan that establishes immediate goals and interventions, based on nursing diagnosis. Includes the patient and the family in the planning process.
Initiate communication with attending physicians, hospice staff members and other facilities as needed to coordinate optimal care and use of resources for patient/family members.
What You Must Have:
Registered Nurse licensed in the state where you work
Valid Driver’s License and Automobile Insurance
Two (2) years of RN and/or Hospice Admission experience in a clinical care setting preferred; hospice experience strongly preferred.
