Responsibilities:
- Developing assessments and written plans of care to identify the needs of each resident.
- Assessing the resident's psychosocial and emotional needs.
- Effectively communicating information concerning a resident’s condition with other departments and government agencies.
- Ensuring that the medically related emotional and social needs of the resident are met and maintained.
- Participating in resident care planning conferences.
- Participating in the resident discharge planning, providing information to residents and families of programs available.
- Serving as a liaison with community agencies to ensure needed ancillary services are available and provided within requested timeframes.
- Making referrals and documenting outcomes.
- A strong commitment to maintaining the confidentiality of all resident care information in accordance with HIPAA Guidelines.
Qualifications:
- Bachelor’s degree is required; focused in Social Work or other Healthcare related fields required.
- The ability to pass a background check is required of all Maryville Employees
- Recent experience in a Social Services role in a nursing home/hospital setting or some other Social Worker role required.
- Ability to work in a fast-paced work environment.
- Excellent communication and customer service skills.
- Strong computer skills required due to this facility working solely in an Electronic Medical Records environment.
Physical requirements:
- Must be able to lift 25 pounds at times.
- Required to stand or walk for prolonged periods.
- Ability to lift, push, pull or move at least 50 pounds independently.
- Requires regular bending, stooping, reaching, and kneeling.
