Palliative Care Coordinator


Qualification and Education Requirements 
  • High School Diploma or equivalent
  • Current Driver's License & Reliable Transportation
  • Must provide ongoing valid and current auto liability insurance
  • Excellent communication and relationship building skills
  • Proven experience in healthcare field or community outreach role
  • Compassionate and empathetic approach to patient care
  • Ability to work independently and collaboratively within a team
  • Experience with EMR software is a plus             

Essential Duties
  • Initiate contact with potential clients via phone, email, and in person meetings
  • Educate potential clients about palliative services available to them through InHome Connect
  • Follow up and maintain an up-to-date record of interactions
  • Navigate EMR efficiently to accurately record and communicate the needs and activities of clients and/or patients
  • Effectively communicates patient needs to the palliative team
  • Schedules patients for visits / phone calls with appropriate team members
  • Acquires consent needed to enroll new patients in palliative care
  • Complies with Medicare, Medicaid, and HIPAA regulations and requirements always.
  • Performs other duties as required.
